Terms and Conditions (Simplified for Patients) – Diverse Minds Clinic

  1. Booking
  • You can book appointments by phone or online.
  • By booking, you agree to our Terms & Conditions and Privacy Policy.
  • Follow-up appointments, repeat prescriptions, and Shared Care Proposals are only available if you’ve already seen a clinician at Diverse Minds Clinic.
  • Booking means you accept our terms. Some services are only for existing patients.
  1. Consultations
  • You can book appointments in advance (depending on availability).
  • We’ll send a confirmation email. If you don’t get one within 12 hours, contact us.
  • Sometimes we may need to reschedule or cancel — we’ll let you know by email.
  • For video consultations, you’ll need to show a UK-issued photo ID (passport or driving licence) before certain prescriptions can be given.
  1. Cancellation
  • Normally, online purchases let you cancel within 14 days (the “cooling-off period”).
  • At Diverse Minds Clinic, you must give at least 72 hours’ notice to cancel or change an appointment. Otherwise, the full fee is charged.
  • If you book less than 72 hours before the appointment, you agree to waive the 14-day cancellation right.
  1. Payment
  • Payment is taken at the time of booking. If it fails, your session is cancelled.
  • You’re responsible for all charges made at booking.
  • Outstanding payments can collect interest (10% above bank rate) and legal costs if unpaid.
  • Fees may change at any time. You’ll pay the price shown when booking.
  • We may pre-authorise your payment card before confirming your booking.
  1. Reports
  • Psychiatric reports are emailed to the address you gave us.
  • If you don’t want your report sent by email, tell us.
  • Reports are usually ready within 10–12 days after your consultation.
  1. Prescriptions
  • Repeat prescriptions may be issued without a review if appropriate.
  • Clinicians only prescribe when it’s safe and in your best interest. Some medicines may not be prescribed online.
  • Prescriptions are not guaranteed — it’s always the clinician’s decision.
  • Prescriptions are private (you pay both our fee and the pharmacy fee). Pharmacy costs are separate.
  • Prescriptions are valid only in the UK.
  • You must use prescriptions only for yourself and follow all instructions. Contact your clinician, GP, or pharmacist if unsure.
  1. Health Insurance
  • Check with your insurer before your appointment.
  • You must pay any charges your insurer doesn’t cover.
  • Insurers don’t cover missed or cancelled sessions — you remain responsible for those fees.
  1. Arrival and Late Attendance
  • If you arrive late, we can’t extend the session time.

         Be on time. We can’t make up lost minutes.

  1. Confidentiality
  • Sessions are confidential.
  • We must break confidentiality if you are at risk of harming yourself or others.
  • Reports are normally sent to your GP unless you request otherwise.
  1. Electronic Medical Records (EMR)
  • Booking creates an electronic medical record (EMR) on our secure Semble system.
  • We normally share your medical information with your GP unless you tell us not to.
  • Semble works on most modern systems but not Internet Explorer or old Edge browsers.
  • Clinicians may sometimes use Skype or other platforms.
  • Technical issues may occasionally suspend services — if they last more than 30 days, you can cancel and get a refund.
  • You’re responsible for checking your EMR is correct during consultations.
  • We keep EMRs in line with UK law, but we are not required to keep all communications forever.
  1. Extra Work
  • Additional letters or reports can be provided, but these are billed separately at our hourly rate.
  • Extra documents cost extra.

  • We do not provide emergency support.
  • If in crisis, call 999, 111, your GP, or the Samaritans.
